Meeting Room Pre-Approval Form
If you have questions about the meeting rooms or this particular service, please contact us first.
Do not request meeting room dates here; this form is used to help authorize you to make reservations. Once your group is pre-approved, only the approved group contact or 2nd contact can request a space and time using our room booking system. If either or both contacts change, please notify the library’s Meeting Room Coordinator.
Note: Contact phone numbers are required and the group contact number will be made public. If no group number is provided a contact’s number will be made public instead.
